- Directly from the client and/or their parent/guardian
- From third parties where The Health Records Act 2001 (the Act) and the Privacy Act 1988 or other laws permit this
The purpose and use of collecting the information:
All information collected by Complete Therapy Group will be:
- To gather and use for the purpose of providing a quality therapeutic service, which may form as part of the assessment, diagnosis and/or treatment you receive. The information is retained in order to document what happens during sessions, and enables Complete Therapy Group to provide a relevant and informed service.
- To communicate with the client and/or their parent/guardian
- To comply with legal obligations
You do not have to give all your personal information, but if you don’t, this may affect the quality of the service you receive or mean the service may not be able to be provided to you.
How may you access the information:
All personal information gathered by the psychologist during the provision of the psychological service will remain confidential and secure except where:
- It is subpoenaed by a court; or
- Failure to disclose the information would place you or another person at serious and imminent risk; or
- Your prior approval has been obtained to:
- To provide a written report and/or client notes to another professional or agency (e.g. GP or a lawyer); or
- Discuss the material with another person (e.g. parent or employer); or
- If disclosure is otherwise required or authorized by law; or
- You would reasonably expect your personal information to be disclosed to another professional or agency (e.g. your treating doctor or other allied health professionals) and disclosure of your personal information to that third party is for the purpose for which is directly related to the primary purpose for which your personal information was collected; or
- Disclosure is otherwise required or authorized by law.
